Policies and procedures that could have prevented a mass shooting that left four students dead at a Michigan high school last year were approved earlier but never implemented, two former school board officials said Monday.

Sixteen-year-old Ethan Crumbley recently withdrew a possible insanity defense and pleaded guilty to 24 charges.

Authorities have said James Crumbley bought the handgun used in the Nov. 30 shooting as a gift for his 15-year-old son Ethan.
